Who needs to complete this form?

The ODC may require informed consent forms for people identified in a licence application who may influence the business affairs of the proposed licence holder. This may include:

  • business associates
  • company directors
  • immediate family members
  • other individuals connected to the applicant or business.

Why the form is required

When assessing a licence application or licence variation, the ODC may seek additional information about people associated with the applicant. Informed consent allows the ODC to obtain and consider this information as part of its assessment process.

Applicant responsibilities

Applicants are responsible for obtaining and submitting completed informed consent forms for any individuals identified by the ODC. The ODC does not contact these individuals directly to obtain consent.

You should ensure all required forms are:

  • completed
  • signed
  • submitted with, or in support of, your application.

Failure to provide consent

Delays may occur if required consent forms are not provided. In some circumstances, failure to submit requested forms may affect the assessment of your application or result in the application being refused.
